Exploring patient choice in GP services

The report presents the findings of a review into how GP services are working for patients, with a specific focus on the role of choice and competition. The review drew on a variety of sources of evidence; a commissioned survey of 3,200 patients, interviews with 25 GP providers and spoke to and gathered information from NHS England, CQC and other stakeholders.

Peer support: what is it and does it work?

This review found evidence that peer support can help people feel more knowledgeable, confident and happy, and less isolated and alone. It also showed that there is a limited understanding of the different forms of peer support, how best to deliver support and the forms of training and infrastructure to get the most impact from it. It concludes that further evidence is needed to fully understand the impact that peer support has on the health service and individuals with long-term health conditions.

Hidden citizens: how can we identify the most lonely older adults?

In 2013, the Campaign to End Loneliness consulted over 100 frontline service providers about what they needed to improve their support. Nearly half said they wanted a tool or information that could help them to identify people experiencing – or at risk of – loneliness. This report explores what was already known in both research and practice about identifying people experiencing loneliness. It looked at current approaches to identifying loneliness and searched for insights into how services can improve their outreach and support.

Moving services out of hospital: joining up General Practice and community services?

Making our health and care systems fit for an aging population

This report sets out a framework and tools to help local service leaders improve the care they provide for older people across nine key components. Within each component of care, the report sets out the goal the system should aim for, presents key evidence about works, gives examples of local innovations, and some pointers to major reviews and relevant guidance. It argues that if the health and care systems can get services right for our older population – those with the highest complexity, activity, spend, variability, and use of multiple services – they should be easier to get it right for other service users.

Towards whole person care

It sets out broad themes on creating a health and care system that is fit for the future. It makes a case for a number of person-centred guarantees at the heart of the vision for whole person care, focused on people with long-term conditions and older people.
